Keratomalacia,or‘melting’,iscornealstromalcollagendissolutionandliquefactionundertheinfluenceofelevatedlevelsoftissue,microbial,andtearfilmproteases.Thekeratomalaciagivesagrayish-gelatinousliquefiedappearancetotheulcerandtheulcermargin.Meltingstromaintheearlystagesmaybedifficulttodistinguishfromcornealedema.Thiscatastrophiccornealdegenerativeconditionmaydeterioraterapidlyandleadtocornealperforationandirisprolapseinasfewas12–48hours.Proteolyticenzymes(proteases)performimportantphysiologicfunctionsinthenormallyslowturnoverandremodelingofthecornealstroma.Naturalproteaseinhibitorsarealsopresentintheprecornealtearfilmandinthecornea.Thesenormallypreventexcessivedegradationofnormalhealthytissue.Meltingcornealdisordersoccurwhenthereisanimbalancebetweenproteasesandproteaseinhibitorsinfavoroftheproteases,thereforecausingpathologicdegradationofstromalcollagenandproteoglycansinthecornea.
